Illinois Insurance Plans Accepted: Blue Cross Blue Shield PPO, Blue Cross Blue Shield Blue Choice PPO, United HealthCare / Optum, Medicare. 

Michigan Insurance Plans Accepted: Blue Cross Blue Shield PPO, Aetna, Medicare.

New Jersey: All services in New Jersey are out-of-network and private pay.

Wisconsin: All services in Wisconsin are out-of-network and private pay.

Out of Network Insurance Benefits: Clients in all states that have out-of-network mental health benefits will be provided with an invoice for submission to their insurance company as an out-of-network claim.   In this case, clients pay privately and request insurance reimbursement.

Fees

Standard 60-minute Session Fee: $200.00

Cancellation Policy

For appointments cancelled with less than 24 hours’ notice, clients will be assessed a $50 fee.

A no-show appointment (no notification provided) will be assessed a $100 fee.
